Conductor Pathway Clinic
The conductor pathway is the clinic model of obtaining permanent access for dialysis. Boise Kidney and Hypertension Institute would like all of our patients to take the following steps: the options class, vein mapping, getting education, getting an access, and starting dialysis.
Options class is an interactive class held in the Boise Kidney & Hypertension Institute Meridian clinic twice a month for 2 hours. It is usually held on Thursdays at 10:00 am. Participants have the options of peritoneal dialysis, hemodialysis, and transplants explained by the healthcare team. This team consists of nurses, dietician, social worker, and PA. Participants are encouraged to bring a family member or friend with them.
Vein mapping is an ultrasound of your arms and possibly your legs to see if you are a good candidate for an arterial venous fistula or if you will need a graft. Vein mapping is done at the vascular laboratory in the hospitals. This is done for everyone even if you have chosen to do peritoneal dialysis.
Appointments can be scheduled with the dietician, the social worker, and the nurses for education and questions that you may have during this process.
You will have assistance with scheduling your appointments with the surgeon’s office for dialysis access placement. You will also have monthly visits with a nurse at Boise Kidney & Hypertension Institute to see how your access is maturing after you have had the surgery. These visits may be phone calls or connected with your scheduled appointments at Boise Kidney & Hypertension Institute.
You will be asked for your consent to have the nurse review your information and use this information to evaluate the process of obtaining dialysis access.
